2024-25 Athletic Forms
ALL fall athletes need to turn in their Athletic Parent Consent/ Physical BEFORE August 19th 2024, to be eligible to participate in tryouts/ practices/ games. Our office will be open M-F starting July 11th. Please call our office @503-980-6284
